Case Study: What Happened When One Client’s AI Visibility Dropped 62% in a Month

A healthcare logistics client of ours saw Google traffic grow 46% in a single month, while their AI-assistant citation rate fell by more than half over the same period. Those two numbers moving in opposite directions is the story every business needs to understand about the current search landscape see how we caught it.

The setup

The client is a regional medical courier company we run monthly geo-grid AI visibility scans for — the same buyer prompts (“best medical courier near me,” “STAT medical courier,” “specimen courier service”) run against ChatGPT and Google AI Overviews across a 49-point grid centered on their home market, repeated monthly so the trend line means something.

What the numbers showed

Month over month, the client’s core “medical courier” prompt went from being named by ChatGPT in 37 of 49 grid points to just 14. Their “specimen courier” prompt fell from 35 of 49 to 6. Google AI Overviews told a similar story: 46 → 33, 43 → 23, and 41 → 9 across their three tracked prompts.

In the same window, their Google Search Console clicks rose from 2,650 to 3,880 (up 46%), impressions rose from 49.1K to 69.2K, and GA4 sessions rose from 4,989 to 7,155 (up 43%). Every traditional SEO metric said the month was a clear win. The AI visibility numbers said the opposite was happening in a channel traditional metrics cannot see.

Why the two metrics diverged

Traditional SEO rewards a slow-moving algorithm that trusts sites which have proven themselves over months and years. AI answer engines re-query the live web for every question, weighing current directory freshness, current review activity, and whichever competitor most recently published a quotable, specific answer. A business can be doing everything right by the old scoreboard and still be losing shelf space in the new one, simply because someone else moved first on the sources the model reads.

What we did about it

We did not touch the SEO work that was clearly succeeding. Instead we treated the AI-visibility drop as its own diagnosis: refreshed directory profiles that had gone stale, added specific, service-named language to the review-request flow, and published additional quotable, question-first content on service pages the model was no longer citing. The client’s next report will show whether that closes the gap — and either way, we would not have known to act without measuring the two channels separately.

The takeaway

If you only track Google Search Console and GA4, a healthy-looking month can hide a real problem in a channel that is only going to matter more. Track AI visibility as its own line, not as a footnote to organic traffic.

Frequently Asked Questions

Why would AI visibility drop while Google rankings improve?

They are measuring different things. Google rankings track how your site performs against a search algorithm that changes slowly. AI answer citations track whether a language model’s live web search still finds and trusts your sources today — and those sources shift week to week.

What usually causes a sudden AI-visibility drop like this?

In this case it was a mix of source churn (older directory listings losing freshness signals) and increased competitive noise in AI answers as more competitors started publishing AI-targeted content. Neither is a site error — it is closer to losing shelf space to competitors who moved first.

How do you actually measure a drop like this?

Geo-grid AI visibility scanning: running the same buyer prompts against ChatGPT and Google AI Overviews across a grid of map points, then comparing the percentage of points where the business is named month over month.

What is the fix once you find a drop?

Refresh the citation layer — directory profiles, review text, and quotable on-site content — and re-measure monthly rather than quarterly. AI visibility moves faster than traditional SEO in both directions.
